What is QuestionnaireResponse?
A QuestionnaireResponse is a FHIR resource that holds the answers a patient or clinician submitted to a Questionnaire, with structured items linked back to the Questionnaire definition.
In context
Each answered item carries a link back to the corresponding item on the Questionnaire definition, the answer value, and any nested groups. That link keeps the response interpretable against its definition and makes scoring, summarisation, and downstream analytics straightforward.
QuestionnaireResponses are typically created in response to a due Task raised by a care-plan-driven schedule, but they can also be created ad hoc, for example, an intake form a patient completes on first signup.
How Fire Arrow handles it
Fire Arrow Server stores QuestionnaireResponses as standard FHIR resources. Patients see their own through the Patient compartment rule. Clinicians see their organisation's responses through the organisation-scoped legitimate-interest rule. A FHIR Subscription on QuestionnaireResponse lets a downstream consumer score the response and raise threshold-based alerts independently of the questionnaire UI.
